ASSESSMENT & SCREENING PROCESS OF SENIOR LIVING COMMUNITY

Every senior living community within our referral network has successfully completed a screening and assessment process and has commited to provide person-centered, accountable care.

SCREENING PROCESS

  • Review of public disclosure files
  • Verification of state-issued license
  • Review of Special Investigation Reports via the licensing department at Department of Human Services (if applicable)
  • Letter of recommendation from a resident or his/her family

IN-PERSON ASSESSMENT

  • Determine transition of care processes/procedures
  • Interview management, staff and/or residents
  • Identify the facility’s strengths and limitations
  • Assess the living environment
  • Determine the level of personal/medical care provided
  • Identify cost, amenities, vacancies
